Witness Information Form

A witness who saw what happened can matter enormously later — but only if you have a way to reach them and a record of what they actually said at the time. This form gives you both. Nothing here is collected or sent anywhere — it's simply a sheet for your own use.

"I'll remember" rarely survives a week, let alone a claim that takes months.

How to use it

If it's safe to do so at the scene, ask any witnesses for their contact details and a brief, plain account of what they saw — before they leave and before memories start to shift. A witness who's easy to reach later, with a note of what they actually said at the time, is far more useful than someone you have to track down weeks afterward.
