A concussion is a mild traumatic brain injury caused by a sudden jolt or impact that makes the brain move rapidly within the skull. In a car accident, this can happen even without your head striking anything directly — a hard, sudden stop can generate enough force on its own. It's one of the more commonly missed car accident injuries, largely because of how differently it presents compared to what most people picture when they hear "brain injury."
You don't need to have lost consciousness
One of the most persistent misconceptions is that a concussion requires blacking out. In practice, most concussions involve no loss of consciousness at all — you can be alert, talking, and seemingly fine immediately after the crash and still have one. This matters because it's easy for both the person injured and anyone around them to assume that being conscious and coherent at the scene rules out a head injury, when it doesn't.
What to watch for
Concussion symptoms are broader than headache and dizziness, and several categories are easy to dismiss as ordinary post-accident stress rather than a brain injury:
- Physical — headache, dizziness, nausea, sensitivity to light or noise, blurred vision.
- Cognitive — difficulty concentrating, feeling mentally "foggy," slower processing, memory gaps around the event.
- Emotional — irritability, unusual anxiety, or mood changes that weren't present before.
- Sleep-related — sleeping more or less than usual, or trouble falling asleep.
These often build gradually over the hours following a crash rather than appearing all at once, which is part of why a person can genuinely feel okay at the scene and only recognize something is wrong later that day or the next.
Why a "normal" scan doesn't rule it out
A CT scan or MRI after a head injury is mainly there to rule out more serious, visible problems — bleeding, swelling, or a skull fracture — not to diagnose a concussion itself. A concussion is typically a clinical diagnosis, made from your reported symptoms and a neurological exam, and it's entirely normal for standard imaging to come back clear even when a real concussion has occurred. Don't take a clear scan as confirmation that nothing happened; it usually just means nothing more serious was found.
Recovery and what's sometimes called post-concussion syndrome
Many people recover from a concussion within one to a few weeks with rest and a gradual return to normal activity. A meaningful share of people, though, experience symptoms that persist considerably longer — sometimes referred to as post-concussion syndrome — and there's no reliable way to predict from general information which path applies to a given case. This is one of several reasons head-injury symptoms deserve ongoing attention rather than a single check-up and moving on.
Documenting it well
- Get evaluated promptly, even if you feel largely fine — a same-day or next-day exam establishes a baseline close to the accident.
- Track cognitive and emotional symptoms too, not just physical ones — difficulty concentrating at work is just as relevant as a headache.
- Follow up if symptoms persist rather than assuming they should have resolved by now.
